Diagnosis and individualized conservative therapy for a disc displacement with reduction: a case report

Abstract

Objective: To diagnose and therapeutically manage the clinical case of an adult male patient with disc displacement with reduction. Case Report: A 36-year-old male patient was referred to the dental clinic of the extension project "Alleviating Orofacial Pain – TMDs and Bruxism" at UFJF-GV, presenting with a chief complaint of pain in the right lower first molar (tooth #46), associated with teeth clenching, as well as reporting joint sounds near the ears. The symptom had begun approximately six months earlier. After a detailed anamnesis, clinical examination (Axis I of the DC/TMD), and analysis of imaging exams, including magnetic resonance imaging (MRI), the patient was diagnosed with sleep bruxism (SB) and disc displacement with reduction (DDwR) in both temporomandibular joints (TMJs). A conservative therapeutic plan was then established, including counseling and the use of a stabilization splint, aiming to protect the temporomandibular joint and relieve pain. After three months of treatment, the patient showed significant pain improvement and better control of parafunctional habits. Discussion: The discussion highlights the importance of an individualized approach to TMD treatment, emphasizing accurate diagnosis through magnetic resonance imaging, a biopsychosocial approach, and control of parafunctional habits. Conclusion: The conservative treatment adopted resulted in a favorable clinical outcome, with pain remission and improved control of parafunctional habits. Therefore, the initial three-month follow-up reinforces the importance of individualized management for therapeutic success in cases of DDwR and SB.
